Monte asked me for a list of the Dead's FM broadcasts, so here is a list up through '78, which is as far as I'll go with this.
Obviously, the majority of FM broadcasts from the early period come from the fall '71 tour.
It's trickier than you might think finding which shows were originally broadcast on FM, since so many shows first came into circulation through David Gans' radio show - so usually when you see an FM source, it came from him.

This list is probably incomplete since I may have missed a few - especially if you include DJs in the '70s playing Dead tapes they got hold of - I listed a couple examples of those.

11/19/66 - was broadcast by KSAN, San Francisco - don't know when, probably recently.
2/14/68 second set - broadcast live by KMPX, San Francisco
[10/12/68 - was broadcast incomplete by KSAN in '70s as "10/13/68", which was the most common source til recently]
4/6/69 - broadcast live on KPFA, Berkeley
[part of 5/2/70 broadcast on FM sometime in '71]
[possibly 5/6/70 on MIT radio station]
5/15/70 - was broadcast by KPFA, Berkeley in 1971 from Fillmore tapes
10/4/70 - broadcast live by KQED and KSAN, San Francisco
[possibly 10/24/70]
12/31/70 - broadcast live by KSAN, San Francisco

Well, I couldn't find station IDs for all the fall '71 shows, but for completeness' sake, these are the ones I could find. (One show per city was broadcast.)

Probably most the remaining shows could be identified by just listening to the old sources for station IDs....but I have to admit, that's a bit too much effort for such a trivial issue!

A friend of mine says that some of the earlier Frost shows were broadcast on KZSU. The 92 or 93 Cal-Expo Rex Foundation was also broadcast on a local Sacramento station. David Gans used some of those tapes on his own shows..

Minor correction/addition -

The 3/31/87 show was broadcast on 93.3 FM, WMMR in Philly, hosted by Pierre Robert. Interestingly (or not) Robert interned at KSAN in the 70s which no doubt formed his style.

93.3 (WMMR) and Q102 (WIOQ 102.1) were the top two stations in the Delaware Valley for both the hippie and 'cool' crowd all through Junior High and High School.

10-21-71 was WGLD - Oak Park / Chicago

. . . and this show was not broadcast live, but rather the tapes of 10/21 were played (including the New Riders' set) in their entirety the following night 10/22 due to "issues with the bands and their record companies ..." or so says the DJ - as he kills time while he changes the tapes 'on-air' after having mistakenly put up the tape of the beginning of the New Riders' set instead of correctly playing the Dead's set two which was due up next. That's what is on my tape that I recorded off of WGLD on 11/22 - 'live'.
